Paul Kotak's Obituary

Paul Kotak, age 79, of Indiana, PA, formerly of Avon, NY, passed away on Thursday, October 1, 2015 at the Punxsutawney Area Hospital. He was born in 1936 in Fulton Run, PA to Joseph Kotak and Mary Blazavich Kotak Sirochman. Paul was self-employed in the automobile tire business for many years in Avon, known locally as K & W Tire. Paul is survived by a daughter Beverly A. Kotak and companion Paul Brisbane, Wayland, NY; five brothers and one sister - Joseph Kotak and wife Jennie, Spearfish, SD; John Kotak and wife Geri, Tonawanda, NY; Rick Sirochman, Rural Valley; Ron Sirochman and wife Kathy, Dayton; Ed Sirochman, Punxsutawney; Irene Gmys and husband Paul, Home; brother-in-law Raymond Wagner, Belfast, NY; sister-in-law, Shirley Sirochman, Home; and many nieces and nephews. He was preceded in death by his parents, by his wife Patricia Bernhard Kotak in 2001, one son Paul E. Kotak in 1992, a step-father John Sirochman, and his brother and sister, Betty Wagner and Steve Sirochman. There will be no visitation. A memorial service will be held Sunday at Noon at the Bowser-Minich Funeral Home, Indiana. Interment will be private.
